Re: Vendor Forms for RPPD 2010

 

The forms that should be returned to me, along with the appropriate Fees, are:

        Merchant’s Waiver Form

        Merchant’s Registration Form

        Monroe County Parks Department Permit Application Form

        Sponsor Form

        Advertising Form

 

 

The “Parks Department” form has been partially filled out for you with the generic information for Rochester Pagan Pride Day.  Please fill in the rest of the information requested as completely as possible.   Make sure that you include both a valid email address and your US Mail address. 

 

Payment for all Fees is being accepted via CHECKS or MONEY ORDERS, for those using snail mail to return their forms, or via PAY-PAL, for those who wish to submit their forms via email.  We are not able to accept payments by credit cards.  Our PayPal address is Paypal@rochesterpaganpride.com

 

   The Monroe County Parks Department will be sending your permit to us as part of our Master Vendor List about 

   2 weeks prior to the date of the festival. 

 

   We will be using this Master List for check-in procedures on Festival Day, so remember to check in with me

   when you get there.
